    Requirements and Prerequisites
    Applicants with a professional degree in pharmacy, medicine, or a health-related discipline or a baccalaureate or masters degree in related fields such as public health, economics, epidemiology, health services, or biostatistics are all encouraged to apply. 

    GRE Requirements
    GRE Requirement: Required. Official GRE scores are required for all applicants in order to be considered for admission. The UW institution code for the GRE is 4854.
    Additional Instructions
    Required materials submitted online include:

    (1) a statement of purpose,
    (2) curriculum vitae or resume,
    (3) three letters of recommendation,
    (4) GRE Scores,
    (5) all university-level transcripts, and
    (6) optional personal history statement.

    Non-Native English Speaker Information
    Graduate School requirement: English language proficiency (ELP) is required for graduate study at the University of Washington. Therefore, every applicant whose native language is not English must demonstrate English language proficiency as outlined in Graduate School Policy 3.2.

    Applicants who will need a visa should reference the International Applicant Information page. If admitted to the program, international students requesting an F1 or J1 visa will be required to provide financial ability documentation to the UW Graduate School.
