Epi PhD Application and Materials Please do not submit applications to multiple degree programs within the Department of Epidemiology. We will only review one application per applicant and will not be able to refund application fees for an individual who submits multiple applications to the Department of Epidemiology. Additional Instructions Recommendation Instructions: At least two, and preferably three, letters should come from individuals with advanced degrees (i.e. PhD or MD). Non-Native English Speaker Information Graduate School requirement: English language proficiency (ELP) is required for graduate study at the University of Washington. Therefore, every applicant whose native language is not English must demonstrate English language proficiency as outlined in Graduate School Policy 3.2.
